Understanding why scar remodelling requires physical intervention is the first step toward understanding what these treatments can realistically achieve.
When the skin repairs a wound — from acne, injury, or surgery — it deposits collagen rapidly to close the breach. This repair-mode collagen is laid down in a disorganised, parallel-fibre pattern rather than the basket-weave architecture of normal dermis. The surface irregularity of a scar is the expression of this disordered sub-surface collagen structure. Until the collagen is remodelled, the surface texture cannot be corrected.
The dermis — where scar collagen resides — is below the stratum corneum and epidermis. Topical actives, including retinoids and AHAs at standard concentrations, do not reliably penetrate to the depth where scar remodelling must occur.
