Understanding why scar remodelling requires physical intervention is the first step toward understanding what these treatments can realistically achieve.
When the skin repairs a wound — from acne, injury, or surgery — it deposits collagen rapidly to close the breach. This repair-mode collagen is laid down in a disorganised, parallel-fibre pattern rather than the basket-weave architecture of normal dermis. The surface irregularity of a scar is the expression of this disordered sub-surface collagen structure. Until the collagen is remodelled, the surface texture cannot be corrected.
The dermis — where scar collagen resides — is below the stratum corneum and epidermis. Topical actives, including retinoids and AHAs at standard concentrations, do not reliably penetrate to the depth where scar remodelling must occur. They improve surface texture by accelerating epidermal turnover, but they do not disrupt or remodel the sub-surface scar collagen that produces the depression or irregularity. Physical micro-disruption at dermal depth is required.
